Skilled Nursing – Expert Nursing care in the comfort of your own home. Services include Heart Failure Patient Care, Outpatient Anti-Coagulation, G.I. Tube Feeding Management, Wound Care and Dressing Changes, Venipuncture for Lab Test, Ostomy Care, Preventing Falls, Rehabilitation Guidance, Wound VAC Therapy, Diabetic Care, Medication Injections, and Pain Management.
Physical Therapy - Rehab Therapy performed by licensed Physical Therapists. Services include Joint Mobility Training, Proper Posture and Positioning, Post Surgery Care, Training in Posture and Positioning, Prosthetics Fitting and Training, Vestibular Restoration/Balance Training, Arthritis Care, and Reduce Falling Tendencies.
Occupational Therapy – Mobility Enhancement and Self-Care Skills Re-education by Occupational Therapists. Services include Coordinating Activities to Promote Independence, Body Mechanics Rehabilitation, Work Simplification, Occupational Therapy Equipment Training, Cognitive and Psychosocial Needs Evaluation, Assessing Client’s Home to Promote Safety, Improving Coordination, Traumatic Brain Injury Treatment, and Enhancement of Body Mechanics.
Speech Therapy – Communication Skills Improvement. Services include Treatment for Motor Speech Disorders, Individualized In-Home Therapy Sessions, Articulation and Pronunciation Exercises, Reducing Phonological Delays, Minimizing Cognitive Delays, Training for Eating and Swallowing Strategies, Involuntary Stuttering, and Treating Fluency Difficulties.
Medical Social Worker – Community Resources, Crisis Intervention and Supportive Administrative Case Work by MSWs. Services include Guidance on Healthcare Policies, Hospital Discharge Assistance, Parenting Guidance on Counseling, Planning Alternative Living or Supportive Living, Providing Guidance on Healthcare Policy, Counseling Sessions with the Patient, Helping Surviving Family Members Cope with Death or Illness, and Facilitating Advance Directives (Living Will or Power of Attorney).
Home Health Aide – AM and PM Care, Assistance with Toileting, Medication Reminders, General Symptom Monitoring, Skin Care and Basic Wound Care, Light Housekeeping, Laundry and Linen Changes, General Safety Supervision, Assistance in Maintaining a Home Exercise Program, Transportation Arrangements to Social Functions, and Running Light Errands; To name a few.
Spiritual Care – We Provide Spiritual Care support to a patient and their family. We will serve as a liaison between them and their religious community. An interfaith chaplain offers spiritual support and pastoral counseling. In addition, the chaplain can assist families with funeral and memorial planning.
Volunteer Services – Volunteers provide additional support. They help with household chores or grocery shopping – any number of things you might ask of a friend. They may stay with the patient so the caregiver can take a break.
Grief Support – Individual grief counseling and support groups for any individuals or team/community who suffered the loss of a loved one. These Services are not limited to those whose loved one received hospice services.
